This is a credit card link for Spiritual Direction. It is important that your Spiritual Director knows you have paid for your Direction session via credit card. Therefore you will be asked to write the recipient's name and email Address. Your Director is the recipient so it is her name and email address you put in that section. Where it says "YOUR INFO" you would put your name so the Director will know which Directee paid for the Session.
